Asthma CHAMP: Helping families manage asthma


Asthma CHAMP is a one-year pilot project funded by the Human Resources Administration (HRA) and targeting Temporary Assistance to Needy Families (TANF) participants who have an exemption from employment activities because of home-care responsibilities for school-age children with asthma or other chronic illnesses. The overall goal of the project is to assist this targeted group of TANF participants to overcome barriers to engaging in educational or work activities through disease management, asthma education, case management, referrals and job readiness activities.

Asthma CHAMP offers home-based asthma case management provided by a Community Health Worker (CHW) that includes:
  1. Education in asthma self-management, trigger avoidance strategies, medication and equipment;
  2. Education in chronic disease management
  3. Care coordination that links families to supportive services including health care access, school-based support and other social services based on an individual family service plan
  4. Job readiness activities including resume preparation, life skills assessment, interviewing and time management skills

Asthma CHAMP is the first HRA-funded project to target families of children with chronic illness and the first to provide one-on-one home based job readiness services through Community Health Workers.
