Greenscreen Form

This is a form for getting placed into CUNY's LDAP directory (e.g., for Blackboard access). Usually this will not be necessary, but if there is a problem with your HR processing, you can fill out this form and bring it to the Accounts Manager at the Faculty/Staff Service Desk on the 4th floor of the Thomas Hunter Building.
