Alter Ego Application

This form should be filled out & returned to the Faculty/Staff Help Desk in HN 109. "Alter ego accounts" are used for: 1. General purpose mailboxes that will be accessed by multiple people within in a department or committee (i.e. Spanish Department Holiday Party Planning Committee), 2. High volume, specific purpose mailboxes (i.e. Graduate English Department Program Inquiries), and/or 3. A titular role that is frequently transferred between individuals (i.e. Math Department Chair).
