Alter Ego Application (Online)
- "Alter ego accounts" are used for: 1. General purpose mailboxes that will be accessed by multiple people within in a department or committee (i.e. Spanish Department Holiday Party Planning Committee), 2. High volume, specific purpose mailboxes (i.e. Graduate English Department Program Inquiries), and/or 3. A titular role that is frequently transferred between individuals (i.e. Math Department Chair).

AV Equipment Daily Request Form
- This form is to request audio-visual equipment for classroom use for a single class session. It should be filled out & returned to the Classroom Technology Sevices, HN room 333.

AV Equipment Long-Term Request Form
- This form is to request the extended use of audio-visual equipment in a classroom. It should be filled out & returned to Classroom Technology Services in HN 333.

Discarded/Obsolete Fixed-Asset Certification
- This form is used to confirm that a given piece of equipment is obsolete, and can be disposed of. It should be filled out & returned to Property Management.

Distance Learning Center Reservation Form
- This form is used to reserve the use of ICIT's videoconferencing facilities. It should be filled out & returned to the Distance Learning Center, at the School of Social Work.

E-mail Address Application
- This form is used to request creation of a faculty or staff Hunter e-mail address. It should be filled out & returned to the Service Desk located on the 4th floor of the Thomas Hunter Building, or faxed to 212-772-5799. (Please Note: This form can be filled out prior to being printed.)

Equipment Disposition Form
- This form is used to notify Property Management of any CUNY-tagged equipment that is being disposed of. It should be filled out & returned to Property Management.

FAS/FIS/SFA User ID Request Form
- This form is used to request access to certain CUNY mainframe systems.

Greenscreen Form
- This is a form for getting placed into CUNY's LDAP directory (e.g., for Blackboard access). Usually this will not be necessary, but if there is a problem with your HR processing, you can fill out this form and bring it to the Accounts Manager at the Faculty/Staff Service Desk on the 4th floor of the Thomas Hunter Building.

Job Request Management System (JRMS)
- This is an online form for requesting student data from the Student Information Management System (SIMS) from MIS. To access this form, one must have an authorized username and password, which is assigned by MIS. Please contact Jann Weinrich, Data Control Coordinator, ICIT/MIS.

LDAP Lookup Tool - Student Access Waiver
- This form is to be filled out if a student requires LDAP lookup access.

LDAP Lookup Tool Access Application
- This form is used to request use of the CUNY Portal tool that permits queries against CUNY's LDAP. It should be filled out & faxed to CUNY CIS.

Mailing List Request Form
- This form is to request the creation of a mailing list on Hunter's LISTSERV. It should be filled out & returned to the Faculty/Staff Help Desk in the Thomas Hunter building on the 4th Floor.

Online Directory Update (Faculty/Staff)
- Use this form to add or update your entry in Hunter's online directory.

PC Hard Drive Certification
- This form is used to certify that any hard drives discarded by the College have been cleaned of all confidential and proprietary data. It should be filled out & returned to Property Management.

Poster Request Form
- This form is to request the poster printing services. Be advised that Classroom Technology Services does not proof read, correct, or edit posters. All posters files must be brought along with this form to Classroom Technology Services in HN 333. The form must be filled out and signed by the faculty sponsor in order for services to be provided.

Student Access to SIMS
- This needs to be filled out by managers who are requesting SIMS access for an employee who is also a student at Hunter. This should be submitted along with the completed SIMS application and FERPA certificate to the Registrar's office HN 217.

VPN Request Form (Online)
- Remote access requires your office computer to be part of the Hunter local active directory. If your computer is not or you are unsure if your computer is part of the local active directory please contact the Service Desk at 212-772-4357 before filling out this form.
