Formbuilder is on online application (separate from the WebCMS) that allows you to build and manage online forms. If your department does not have a formbuilder site, you should see this page.
Upgrade of legacy Formbuilder sites
As of October 2014, all Formbuilder sites have been upgraded to the latest version of the software. There is no need to request an upgrade now.
Addition or removal of users for legacy formbuilder sites
All sites have been upgraded to the latests version of the software, which enables self-management of users. You do not need to request addition/removal of users in the future, simply speak to the others within your organization to request access.

An interruption affecting ApplyOnline, FAP Backend, SFA, SKAT, CUDLS, CUNYSupplements, FACTS, FAS, PETS, CAS, eProcurement was completed and lasted from 02/21/2015 02:00 am to 02/22/2015 02:00 pm.
