Application:
The Urban Public Health administrative code is 618 and specialization
code 040. Hunter's institution code (for GRE/TESOL submission) is
2301.
Before
registration is permitted, matriculated and non-matriculated students
must also submit the New York state required Immunization Form downloadable
from the Registrar's website under Forms: registrar.hunter.cuny.edu.
The form can be included with the application, or, once active in
the Hunter registration system, faxed or brought to Hunter Health
Services. Applicants who have an alternative form which includes the
required information may submit a copy of the alternative form. Please
note that all students enrolled in Hunter will be issued a Hunter
email address and password (the password can be changed.) When received,
please supply this email to the UPH office. This email wiill be used
for all communications.Student should also inform Hunter Admissions
and the UPH office of any address or telephone changes which
occur after submission of the application.
