Advanced Standing Core Competencies

OYR Agency Agreement Form for the Mental Health Grant Only
Applicants applying to this program, must have two year post-baccalaureate experience. Working full-time at a social service agency. You must have DOHMH appoval before applying for the grant and paying the application fee. Click here to download Adobe Acrobat PDF

Transfer of Credit Form
This form must be accompanied by a course description and syllabi for each course (you are requesting credit), and an official transcript. Deadline for Fall is April 1, and November 1, for Spring. Please keep in mind, only Advanced Standing students receive credit for undergraduate couses. Click here to download PDF document.
