Lost SEIF & Off-Campus Trip List Request & Appliance Rental Request

Please submit your request for a lost or misplaced SEIF, Off-Campus trip student list, 501c3, etc by completing the following form. Please allow at least 3 business days for processing. Appliance rental requests have been suspended until further notice.

Lost Document Request 

Approved event forms and event documents are available for pick up in OSA(Room TH202). If you have lost a form or require additional copies, you may request a replacement copy of an SEIF, Off-Campus trip student list, 501c3, etc. Please complete the request form and allow at least 3 business days for processing.

Appliance Rental

The Office of Student Activities has appliances available for student club events. Officers of currently chartered clubs may submit an Appliance Rental Request.
