In order to participate in athletic or recreation programs, all students, faculty, and staff must have a physical and a completed Physical Activity Form. These forms are available in the Office of Health Services or you can print it here. These cards are valid for two years and then must be updated. Additionally, a doctor's stamp is required for the form to be accepted.
